Before you book
- How soon can I be seen?
- It depends on the office and the week. If your situation is urgent, or you already have a surgery date booked elsewhere and want a second opinion first, say so in the message. It changes how the appointment is scheduled.
- Do I need a referral?
- It depends on your plan rather than on the practice. Some plans require a referral or prior authorisation for a specialist visit and some do not. The office will check when they contact you, or you can call your insurer beforehand.
- What should I bring?
- Your imaging on a disc or a share link rather than only the report, photo identification, your insurance card, a medication list, and any prior operative notes. Your first visit covers the full list and why the imaging matters most.
- Which location should I choose?
- Whichever is most practical. If you are not sure, leave it as no preference and say where you are travelling from. The areas we serve page also maps communities to the nearest office.
- Can I send my imaging before the visit?
- The office will tell you how to send records securely when they contact you. Please do not send medical records or images through this form, which is not a secure channel.
- Is this form monitored for emergencies?
- No. This form is not monitored continuously and is not for urgent problems. If you have new or worsening weakness, loss of bladder or bowel control, or numbness around the groin, go to an emergency department or call 911.
Worth doing now
Request your imaging.
If your films are held at another hospital, getting a copy can take a few days. Starting that before your appointment is the single most useful piece of preparation, and it is the thing most often left too late.
